Freedom of Information

Information about the General Practitioners and the practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the Practice Manager.

Access to Records

In accordance with the General Data Protection Regulation and the Access to Health Records Act, patients may request to see their medical records or request a Subject Access Request (SARS)

Any requests should be made in person by completing a Subject Access Request form and bringing valid photo ID. Such requests may be subject to an administration charge for producing/printing documents. No information will be released without patient consent (apart from exceptional circumstances when legally obligated to do so).

If you would like to request this via email, you can send your request to b83052.kensingtonstr@nhs.net – however you still may be required to complete a practice Subject Access Request form, provide valid photo ID and pay any administration charges for producing/printing documents.
